Our responsibilities
Victor J. Dongo, DMD, PA is required by law to maintain the privacy and security of protected health information, provide this notice of our legal duties and privacy practices, follow the notice currently in effect, and notify affected individuals following a breach of unsecured protected health information as required by law.
How we may use and disclose health information
- Treatment: to provide, coordinate, or manage dental care and consult with other healthcare professionals involved in your care.
- Payment: to bill and collect payment, confirm coverage, submit claims, and obtain prior authorization where applicable.
- Healthcare operations: for quality review, training, licensing, auditing, business planning, and other activities needed to operate the practice.
- Other permitted or required uses: when required by law or permitted for public health, oversight, legal proceedings, law enforcement, workers' compensation, emergencies, preventing serious threats, and other purposes allowed by applicable law.
Uses that may require authorization
Uses or disclosures not otherwise permitted by law generally require your written authorization. You may revoke an authorization in writing, except to the extent we have already acted in reliance on it. Certain uses involving marketing, sale of protected health information, psychotherapy notes, or other specially protected information may require specific authorization.
Your rights
- Inspect or obtain a copy of designated health and billing records, subject to limited exceptions.
- Ask us to correct or amend information you believe is incomplete or incorrect.
- Request restrictions on certain uses or disclosures; we are not always required to agree.
- Request confidential communications by a reasonable alternative method or at an alternative location.
- Request an accounting of certain disclosures and a paper copy of this notice.
- Choose someone with legal authority to act for you.
Website communications are different
Protected health information maintained in your dental record is governed by this notice. General website appointment forms, concierge chats, smile-scan uploads, and ordinary email are intended only for limited, non-sensitive communications and may not be configured as secure clinical messaging.
Do not use those channels to send medical histories, diagnoses, records, insurance identifiers, payment information, or urgent health concerns. Call 954-869-0200 for assistance. Our separate Website Privacy Policy explains website data practices.
Complaints and questions

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. Beacon Dental will not retaliate against you for filing a complaint.
Changes to this notice
We may change this notice and make a revised notice effective for health information we already maintain and information we receive in the future. The current notice will be available through the practice and on this website.
